Non-Surgical Options for Cranial Cruciate Tears in Dogs

Does Every Torn Cruciate Ligament Need Surgery?
If your dog has been diagnosed with a cranial cruciate ligament (CCL) tear — the canine equivalent of a human ACL injury — chances are your veterinarian has already mentioned surgery. It's the most commonly recommended treatment, and for good reason: it's well studied and often effective.
But it's not the only option supported by veterinary research — and for some dogs, it may not even be the best one.
What the research actually shows
A landmark study following 85 dogs with confirmed cruciate ruptures found that when managed conservatively — leash-only activity, weight loss where needed, and pain control — nearly 86% of dogs under 15 kg (about 33 lbs) were clinically normal or improved after an average follow-up of three years. Outcomes were less favorable for larger dogs managed this way alone, which is one reason rehabilitation-focused conservative care, not just "rest and see," has become the more rigorous modern approach.
More recent comparative research backs this up. A 2024 study of small-breed dogs under 10 kg found that dogs treated non-surgically improved just as much, in the short term, as dogs treated with tibial plateau leveling osteotomy (TPLO) surgery — one of the most common cruciate surgeries. Neither pain scores nor overall function differed significantly between the two groups.
A systematic review comparing conservative management to a common surgical technique (lateral fabellotibial suture) similarly found no statistical evidence that surgery led to a faster recovery.
Where rehabilitation comes in
None of this means surgery is unnecessary or wrong — for many dogs, especially larger and more active ones, it remains the recommended path. What the research does show is that structured, professionally guided rehabilitation is a meaningful part of good outcomes either way — before surgery, after surgery, or instead of it.
A 2023 review of cruciate injury management noted that physiotherapy protocols address the pieces conservative rest alone doesn't: restoring pain-free range of motion, rebuilding the muscle that supports an unstable joint, and correcting the proprioceptive (balance and body-awareness) deficits that come with injury. A separate randomized controlled trial found that a 12-week progressive home exercise program measurably improved weight-bearing, balance, and stifle function in dogs with stifle injuries.
What this means for your dog
Every cruciate injury is different — the dog's size, activity level, degree of instability, and overall joint health all matter. The best way to know whether a structured rehabilitation program could be a good fit is a hands-on evaluation.
